The Club Constitution Template is a Microsoft Word document that can be downloaded and saved.

Please retain all the materials in the packet prior to the application.

Note: When completing the New Club Formation Packet, you only need to hand in the following:

• The application
• The Notice of Intent Permit
• The Constitution

Please pay close attention to due dates as some forms need more than two weeks to process
